Your First Visit

Your first visit  with Dr. Fatkin at Henderson Foot and Ankle Center establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give Dr. Fatkin time to become familiar with you and your foot problem.

We encourage you to save time at your first visit by printing out and completing the New Patient Forms in advance of your appointment.  Our New Patient paperwork can be downloaded as a PDF by clicking below.

If you are a new patient, we encourage you to print and fill out all New Patient Forms so we can expedite your first visit:

What to Bring With You

Being well-prepared for your appointment will ensure that Dr. Fatkin has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling.

Please bring the following to your appointment:

• Insurance Cards
• Photo ID (Driver's License Preferred)
• Physician | Insurance Company Referral
• List of Medications
• x-rays, MRIs or other studies
• Blood Lab Test Results

You may also consider bringing:

• shoes related to your condition
• orthotics (arch supports, inserts)
• ankle braces

The American Podiatric Medical Association lists the following recommendations as you prepare for your first visit to our office:

• Make a list of your questions, any symptoms, current medications and prior surgeries.
• Compile all of your relevant lab test results and medical records from other doctors and bring them with you.
• Check with your insurance company to see what is needed, and let our office know if you have any special needs.
• Ask a loved one to come with you.
• If you have a problem walking or exercising, bring your workout shoes with you.

Appointments

Although it is preferred to schedule your appointment in advance... emergencies do happen.  For this reason, we will attempt to  accommodate same day appointments whenever possible.

Please arrive at least 15 minutes before your appointment time.  However, If you haven't filled out the New Patient Forms yet, please arrive about 30 minutes early to allow for the extra time needed to complete the patient information.
